The word “program” refers to any education initiative conducted by Qiology or their associated presenters, including but not limited to live-webinar, recorded-webinars, live-seminar, USB classes, etc.

The Course Materials, slides, diagrams & other teaching materials (hereinafter “Course Material”‘) used in the programs are Copyright© of the respective Authors &/or Presenters (hereinafter “Author”).

Anyone who is interested in utilizing the Course Materials in any way other than as learning materials for Qiology’s programs shall contact the Author(s) for SPECIFIC WRITTEN PERMISSION by THE AUTHOR. Anyone who is interested in utilizing the Course Material in any way other than as learning materials for the particular program that it is meant for shall contact the Author(s) for SPECIFIC WRITTEN PERMISSION.

The name of the author(s) (alone or as part of another name) or any logos, seals, insignia or other words, names, symbols or devices that identify the author(s) may not be used to endorse or promote products derived from the Course Material WITHOUT SPECIFIC PRIOR WRITTEN PERMISSION.

2. Non-recording/Non-download agreement:

No recording, storage or download of any Qiology Program by any means, audio, video or other, is permitted by attendees/participants, or those acting on the behalf of attendees or participants of this program. Downloading of the course notes and specific documents provided in the download section of the programs is however permitted.

3. Non-teaching agreement:

Any material that is presented in Qiology programs is not permitted to be used as teaching materials by the participants except with the express permission of the copyright holders. No permission is granted to attendees/participants of this program to use the material to provide educational events of any kind.

4. Consent to Audio/Video Recording by the organization/business, Qiology (not applicable on pre-recorded programs):

As an attendee and participant of Qiology programs (especially those that are live or in webinar format), there is a potential that your name, image, video, voice or likelihood may be displayed, recorded or transmitted during these programs. For live programs, the programs may be fully video recorded by Qiology.

You, as an attendee/participant, understand that these programs will be audio/video recorded by Qiology and/or respective presenter(s) for the purposes of documentation, education, research, promotion and sales. You give authorisation and consent for video and audio recording and use of this program. You understand that all or part of these recordings may be published in electronic format and used at the discretion of Qiology.

You agree that if you do not wish to be recorded, you will inform us before the program commences so that we can make necessary arrangements.

Should you wish a section of the recording featuring you to be removed from the recording after the program is already concluded, you must write to Qiology within 7 days of the completion of the program.

Please note that Qiology reserves the rights to charge you for the additional cost of such removal if requested after the program is already concluded.

In case the participants were used as a demonstration patient during the program, they agree that they do not have a right to request their video, image, audio, name or likelihood to be removed from the program’s recording as it may hamper the educational use of the recording.

5. Release about Burning or Other Side Effects of Moxibustion and Similar treatments (applicable for programs involving moxibustion)

The signatory student/attendee declares to be informed and in accordance with the following aspects:

The practice of moxibustion in its different modalities (direct or indirect) can lead to different side effects, some global, such as tiredness, dizziness and general malaise and in some cases vagal (neurological) reactions, and some local reactions such as irritation of the skin and/or other tissues, burns, blisters and scars.

The student/attendee agrees not to take legal action against other students/attendees, the instructor and the organiser (Qiology) of any program in relation to possible injuries caused by the application of the practices taught during the programs, with the understanding that the practices are in a quid pro quo  relationship (meaning that the student receives the techniques and in the same way performs them on themselves or others) or as a volunteer.

The practices are formative and experiential in nature, and they will not be considered clinical treatments during the program.

The student/attendee should exercise reasonable care towards themselves and any patient or volunteer receiving treatment in case they suffer any pathology (such as clotting problems, cancer, infectious disease, or other) or situation (fever or pregnancy or other,) that could be contraindicated for certain moxibustion techniques practised during the program.

In case of doubt, ask the instructor or the organiser (Qiology) at your first opportunity via email.

The student/attendee assumes their absolute responsibility in the exercise of their acts during the practices section of a Qiology program.

6. Live Seminars

(The following section is applicable to Live Seminars only and  for programs involving demonstration and/or practice of treatments including but not limited to acupuncture, moxibustion, cupping, electrical stimulation, Tui Na (Chinese Massage), Chinese herbal medicine, structural balancing, and nutritional counselling hereinafter referred to as “treatment”)

Release about receiving treatment as a demonstration patient during Live programs and possible effects of treatments.

The signatory student/attendee declares to be informed and in accordance with the following aspects:

7. Informed Consent to treatment (for live program demonstration patients only):

I hereby request and consent to the performance of acupuncture treatments and other procedures within the scope of the practice of acupuncture on me by the acupuncturist/presenter indicated below.

I understand that methods of treatment may include, but are not limited to, acupuncture, moxibustion, cupping, electrical stimulation, Tui Na (Chinese Massage), Chinese herbal medicine, and nutritional counseling. I understand that the herbs/teas/supplements may need to be prepared and the herbs/supplements/teas must be consumed according to instructions provided orally and/or in writing. The herbs may have an unpleasant smell or taste. I will immediately notify the presenter of the organiser (Qiology) of any unanticipated or unpleasant effects associated with consumption of the herbs/teas/supplements.

I have been informed that acupuncture is a generally safe method of treatment, but that it may have some side effects, including bruising, numbness or tingling near the needling sites that may last a few days, and dizziness or fainting. Burns and/or scarring are a potential risk of moxibustion and cupping, or when treatment involves the use of heat lamps. Bruising is a common side effect of cupping. Unusual risks of acupuncture include spontaneous miscarriage, nerve damage, and organ puncture, including lung puncture (pneumothorax). Infection is another possible risk, although the practitioner/presenter uses sterile disposable needles and maintains a clean and safe environment.

I understand that while this document describes the major risks of treatment, other side effects and risks may occur. The herbs and nutritional supplements (which are from plant, animal and mineral sources) that have been recommended are traditionally considered safe in the practice of Chinese medicine, although some may be toxic in large doses. I understand that some herbs may be inappropriate during pregnancy. Some possible side effects of taking herbs are nausea, gas, stomachache, vomiting, headache, diarrhoea, rashes, hives, and tingling of the tongue. I will notify the presenter/Qiology program staff member who is caring for me if I am or become pregnant.

While I do not expect the program staff to be able to anticipate and explain all possible risks and complications of treatment, I wish to rely on the Qiology program staff to exercise judgement during the course of treatment which the Qiology program staff thinks at the time, based upon the facts then known, is in my best interest. I understand that results are not guaranteed.

I understand the presenter and Qiology program staff may review my patient records and lab reports, but all my records will be kept confidential and will not be released without my written consent.

By voluntarily signing below, I show that I have read, or had read to me, the above consent to treatment document, have been told about the risks and benefits of acupuncture and other procedures, and have had an opportunity to ask questions. I intend this consent form to cover the entire course of treatment for my present conditions, and for any future conditions(s) for which I seek treatment during this Qiology program.

I give consent to Palpation for Diagnostic & Treatment Purposes, I give consent to receive Treatment (Acupuncture + Cupping + Gua Sha, and other interventions as listed elsewhere in this document.).

I give consent to receive Moxibustion and have been outlined of the possible risks associated with Moxibustion.

I give consent to receive Acupuncture and have been outlined of the possible risks associated with Acupuncture.

I am aware that informed consent is fluid, and consent may be changed at any moment during treatment and it must be communicated if I do not wish to go ahead with a procedure or treatment before, during or after it occurs.
